I confirm the cat is not showing unexplained vomiting, diarrhoea, coughing, sneezing, parasites or other signs of infectious illness. I authorise necessary veterinary advice and treatment if I cannot be contacted, subject to the signed veterinary-consent terms. I consent to movement to the separate isolation facility and separation of sharing cats where reasonably necessary for welfare or infection control. I have read and accept the booking terms, cancellation policy, owner acknowledgement and veterinary consent.
